Any sample being handed in will require an information slip to be completed. This asks for patient's name, date of birth, telephone number, date & time the sample was taken, which clinician requested the sample and what the sample is for e.g. infection, pregnancy etc.
Please call the surgery to be advised of your sample result. Information on calling for test results can be provided in section Test Results - Samples or Investigations in the Key Information sections.
Where possible we ask that you hand samples in before 1pm.
Please note: If the sample has not been requested by a GP or nurse it may be disposed of. If you think you have an infection please make a GP appointment to discuss your symptoms.
